Work Hours/Shift:
Full Time/M-F
You will be responsible for:
* Responsible for the development, implementation and continued maintenance of a comprehensive review of antimicrobial usage. These responsibilities include the coordination and dissemination of drug information regarding the use of antimicrobial drugs and management of antibiotic utilization.
* The Infectious Disease Clinical Pharmacist will serve as a pharmacy liaison to Antibiotic Stewardship Committee, Infection Control, and Infectious Disease Physicians at FHT; while working closely with the Microbiology Department to evaluate antibiotic susceptibility and resistance patterns.
* The Infectious Disease Clinical Pharmacist is also responsible for the Department of Pharmacy Services staff development program with regards to Infectious Diseases and Antibiotic Utilization.
* Actively participate in therapeutic drug monitoring programs (i.e, drug utilization review, drug use evaluations, adverse drug reactions programs, therapeutic substitutions and antibiotic management programs) specifically for antibiotics.
* Participation in clinical rounds or perform chart review throughout the organization to provide therapeutic antibiotic monitoring.
* Develop policies and procedures as well as institutional guidelines and protocols governing antimicrobial drug administrations and use and drug therapies.
* Create, organize and implement policies/protocols and programs for Antimicrobial Stewardship throughout the institutions.
* Prepare reports on the progress, outcomes, and/or success of the antimicrobial stewardship for submission to the hospitals Quality Improvement Committee, Pharmacy & Therapeutics Committee, Infection Control Committee, and other committees as needed.
* Provide Drug Information and or education programs to local physicians, hospitalists, house staff, pharmacists, and/or other healthcare professionals on baseline information regarding antimicrobials and current utilization practices.
* Serves as a member and active participant in the Antimicrobial Stewardship Committee, Infection Control, Pharmacy and Therapeutics Committee, and other committees as needed.
Qualifications
What you will need:
* Minimum of Bachelor's degree, Doctor of Pharmacy or other advanced pharmacy degree preferred.
* PGY1 & PGY2 Pharmacy Residency in specialty area consistent with practice needs of institution OR PGY1 Residency plus specialty practice experience of 3+ years consistent with practice needs of institution, with portfolio documenting sustained contribution to practice OR Clinical Pharmacy experience in specialty practice of 5+ years in a contemporary hospital setting, with portfolio documenting sustained contribution to practice required.
* FL Pharmacist license.
* Board of Pharmaceutical Specialties certification.
* ACLS from American Heart Association.
