Key Responsibilities:

1. To provide leadership and direction to Behavioral Health Workers and LPNs
2. To assess patient/resident care.
3. To administer medication and medical treatment as prescribed by a physician.
4. To provide and direct milieu management.
5. To document nursing care, treatment progression and implement care plan.
6. Implement nursing and milieu care.
7. Initiate and account for special treatment procedures.
8. To assess, direct, and intervene in crisis situations.
9. Performs other duties as assigned.
10. May perform intakes and receive patients afterhours.
11. Other duties as assigned.

Performance Standard:

1. Completes admission and nursing assessment of patient within 8 hours of admission. Identifies nursing diagnoses and initiates Master Treatment Plan with patient's signature. Charts audited by Director of Nursing for accuracy of assessment and completion of MTP, 90-95% meets and/or exceeds standard.
2. Daily, ongoing assessment and documentation of patients' progress includes initial status and changes in safety, mental, physical, and social function. Documentation is clear, concise, legible, and completed on time. Director of Nursing to randomly select and review 2 records every 2 months, 90-95% accuracy meets and/or exceeds standard.
3. Administers medication according to hospital policy and procedure and physician's orders:
a. Monitors vital signs and lab work for appropriate parameters.
b. Checks medication labels for expiration date and correctness of directives.
c. Administers medication within one hour of prescribed time.
d. Charts all medication administered; documents response to PRN medications. Documents any adverse reaction to medication
e. Disposes of medication according to hospital procedures.
f. Zero (0) incidence of medication variance will earn a 4 rating.
1-2 variances exceed standards, rating 3.
3-4 variances meet standards, rating 2.
>4 variances do not meet standards, rating 1.
4. Notes physician's orders accurately and in a timely manner. Reads back physician's telephone orders for accuracy. Charts audited for T.O.R.B. have 100% compliance. Completes Discharge Instructions accurately and in a timely manner. Completes 24 hour Nursing Report and Admission/Discharge Log accurately.
5. Participates in the multidisciplinary treatment team. Attends staffing when possible. Document patient's progress on the treatment plan review, signs treatment plans when applicable. Participates in weekly treatment team meeting. 90% attendance as documented by signature on attendance record meets and/or exceeds standard.
6. Provides appropriate intervention in crisis, and directs crisis situations using sound judgment and timely response. Accurately completes all Special Treatment Procedure paperwork, including physician's orders and STP log. Standard to be measured by physician, nursing supervisor, and peer report. 95% accurate completion of documentation meets and /or exceeds standard.
7. Provides direction and leadership of LPNs and BHTs. Monitors for completion of assigned duties. Supervises all co-workers and provides constructive feedback to improve quality of care given. Standard will be measure by completion of assignment sheets and peer feedback. 95% completion meets and/or exceeds standard.
8. Demonstrates knowledge of growth and development for the skills necessary to provide appropriate care for all age groups. Demonstrates effective age-appropriate communication, limit setting, interventions, goals and objectives for each age group.
9. Follows all hospital and departmental policy and procedures.
10. Licensure/Certification, Training and Employee Health: (If applicable)
a. Licensure is kept current with no days of expiration and/or suspension. Has provided HR with a copy.
b. Completes CPR and re-certification training on time (prior to expiration date)
c. Complete annual training requirement before December 31st of each calendar year. Has provided HR with a copy of all CEU and/or outside training hours.
d. Current TB with no days of expiration.
11. Attendance: Employee will have less than 4 occurrences this evaluation period.
Customer Service:
1. Develops cooperative relationships with supervisors, peers, physicians, other staff and referral sources.
2. Develops positive relationships with patients and families and responds to their needs/concerns efficiently and satisfactorily.
3. Utilizes systematic problem solving steps to identify and resolve problems/complaints.

Qualifications

Education Requirements: Graduate of an accredited school of nursing. Prefer BSN

Experience: One-year inpatient psychiatric experience preferred.

Licensure Requirements: Unencumbered Registered Nurse license.
