Southwest Medical, part of OptumCare, is a multi - specialty group of physicians, Nurse Practitioners, and Physician Assistants consisting of over 300 providers, 21 medical health centers including six urgent care clinics, four convenient care centers, two lifestyle centers catering to older adults, an outpatient surgery center, home health, hospice and palliative care services, plus E - visits and online options via Now Clinic telemedicine technology. Southwest Medical Home Health, a home health division of OptumCare / United Health Group, offers flexible shifts and the opportunity to make a difference in the care of patients in their home setting.

Primary Responsibilities:

* Evaluate patients' functional status (muscle function, endurance, visual coordination, written and self - care ability, work capacity, etc.). Evaluate home environment for hazards or barriers to more independent living. Identify equipment needs. Participate in the development of the total plan of care
* Develop treatment program and establish goals for improved function. Communicate plan of care to referring physician and other Agency professionals
* May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury
* May instruct patient in the use of various types of adaptive equipment, durable medical equipment, splints, etc.
* May train patient in the use of prosthetic and orthotic devices
* Maintain appropriate clinical records, clinical notes and report to the physician any changes in the patients' condition. Submit these records in accordance with Agency policy
* Maintain contact/communication with other personnel involved in the patients' care to promote coordinated, efficient care
* Identify patient and family / caregiver needs for other home health services and refer as appropriate
* Instruct patients' family / caregiver and other Agency healthcare personnel in patients' treatment regime when appropriate
* Participate in peer consultation process
* Other responsibilities as assigned

Required Qualifications:

* Current unrestricted Nevada occupational therapist license
* Graduate of an accredited occupational therapy program
* Bachelor's degree or master's degree in occupational therapy
* 2+ years of experience as an occupational therapist
* CPR certification or willing to obtain within 30 days of hire
* You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.
* Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.

Preferred Qualifications:

* Home health experience
* Electronic documentation and records experience
* Experience working with geriatric patients
* Experience working with and supervising COTAs
* Experience with Oasis discharges
